In Ayurveda, the first prescription is often the menu. The Pathyāpathyānighaṇṭa is a whole manual built on that idea — an organized list of what is wholesome (pathya) and what is unwholesome (apathya) for each disease, from fever to gout to abscess. Its index alone reads like a table of contents for the clinic.

A diet manual organized by disease

The manuscript's opening index is a marvel of practical organization: for every condition, two columns — the foods that help and the foods that hurt.

Index of the Pathya-Apathya Nighantu: Wholesome foods in fevers, in diarrhea disease, in pandu disease, in hoarseness disease. Fever, chronic fever, irregular fever, agantuja fever — each has its own dietary chapter. The tradition did not treat 'fever' as one thing, and its diet tables follow the same discrimination.

Vitality and what depletes it

Beyond the lists, the text pauses to define its concepts carefully — among them the condition of destroyed vitality, oja-kṣaya, where the body's essence leaks away.

One from whom vitality falls away due to the simultaneous increase of bile and wind, with trembling of the body and confusion, lying down with consciousness lost, even while sleeping that creature is born, and is drowsy and delirious, with bristling hair and frightened limbs, with mild fever and pain — the wise physician should know this as vitality-obstructed.

Diet as the first medicine

The Pathyāpathyānighaṇṭa makes explicit what runs through the whole tradition: food is not separate from medicine. The same care given to a formulation — proportions, timing, contraindications — is given to the daily meal, and the manual gives a physician the tool to prescribe both. It is a dietetics that is also a diagnostic map, organized one disease at a time.
